Description: Fields not showing as per selected template

Steps to follow:
1. Go to waste collection
2. Click on add button
3. Select against (direct)
4. Select template from dropdown
5. observe the screen

Expected: Upon template selected, fields to enter location, bin no, bin weight should display.
